Complex regional pain syndrome (CRPS) is a debilitating neuropathic pain condition that has been extensively reported in the extremities following variable degrees of nerve trauma. CRPS has rarely been reported in the orofacial region. We report 2 orofacial pain patients whose clinical phenotypes fit the criteria for CRPS. Two cases of orofacial complex regional pain syndrome (CRPS) are described, both of which began following trigeminal nerve trauma. In case 1 the patient presented with redness of the ipsilateral ear during painful episodes, pain that extended into the ipsilateral arm and was associated with variations in the appearance of the ipsilateral hand. Symptoms also included “electric-burning pain” of the right side of the head, including the ear, teeth, jaw, eye, neck, and cheek. In case 2 the patient presented with intractable pain of the upper left face, head, and neck accompanied by color changes in the painful areas, which increased with exposure to cold.

We report a case of lymphangioma circumscriptum (cavernous lymphangioma with epithelial hyperplasia) in a 12-year-old girl, presenting as a papillary tumor on the right dorsal side of her tongue. Microscopic examination found cavernous vascular channels lined by a single layer of CD31(+), podoplanin-positive, CD34(-) endothelial cells that occupied the papillary lamina propria and were accompanied by epithelial hyperplasia. A review of the literature on oral vascular tumors with epithelial hyperplasia, namely, lymphangioma circumscriptum and angiokeratoma, provided information that draws into question the use of these terms.

Oral premalignant and malignant lesions may mimic oral lichen planus (OLP) clinically and microscopically. OLP often shows basement membrane fibrinogen positivity on direct immunofluorescence testing (DIF). This study examined fibrinogen positivity in oral premalignant lesions and squamous cell carcinoma.

Necrotizing gingivostomatitis and osteonecrosis associated with antithyroid drug propylthiouracil therapy

A 43-year-old Chinese female had been diagnosed with hyperthyroidism 15 years ago. She was recently administered 150 mg/day propylthiouracil (PTU). After 3 weeks of PTU administration, she developed necrotizing stomatitis and osteonecrosis, most likely due to secondary effects from the PTU treatment. Her neutrophil count was reduced below normal to 0.24×10(9)/L but normalized after withdrawal of PTU therapy. About 1 month after onset, the patient came to our hospital and began to receive intravenous treatments of metronidazole and amoxicillin. Following review of her medical history and a series of clinical and laboratory examinations, the patient was diagnosed with secondary necrotizing gingivostomatitis and osteonecrosis possibly associated with PTU-induced agranulocytosis. One-year after treatment, the patient’s oral manifestations remained unchanged. This case demonstrates the need for dental practitioners to more closely monitor oral symptoms in patients with hyperthyroidism treated with antithyroid drugs.
