Aortic mural thrombus in a nonaneurysmal minimally atherosclerotic or normal aorta is a rare clinical entity and an uncommon cause of peripheral arterial embolization. Both anticoagulation therapy and aortic surgery are commonly used as primary treatment, but there are no consensuses or clinical guidelines to outline the best management strategy for this unusual problem. This systematic review compares the outcomes of these different strategies for the treatment of aortic mural thrombus.

Endovascular abdominal aortic aneurysm repair (EVAR) in patients with unfavorable proximal seal zones remains challenging. The purpose of this study was to identify the incidence of proximal extension cuff usage for type I endoleaks in patients with abdominal aortic aneurysms and unfavorable necks treated with the C3 Excluder repositionable endoprosthesis compared with the traditional Excluder stent-graft.

Chronic mesenteric ischemia: critical review and guidelines for management

CMI is caused by chronic occlusive disease of mesenteric arteries. In such an uncommon disease, clear recommendations are strongly needed. Unfortunately, treatment options for symptomatic CMI are still controversial and no guidelines exist.

Recanalization rates after acute deep vein thrombosis: A single-center experience using a newly proposed vein diameter variation index

Duplex ultrasound scanning (DUS) is the method of choice for diagnosis of deep vein thrombosis (DVT). However, only a few studies have performed prospective serial DUS after an acute episode of DVT to assess its evolution. This study aimed to report our experience using DUS combined with a thrombosis score (TS) and a newly proposed vein diameter variation index (VDVI) to evaluate the rate of resolution of DVT by assessing and quantifying the early stages of vein recanalization in proximal vein segments within 6 months after an episode of acute lower extremity DVT.

When judging the success or failure of major lower extremity (MLE) amputation, the assessment of appropriate functional and quality of life (QOL) outcomes is paramount. The heterogeneity of the scales and tests in the current literature is confusing and makes it difficult to compare results. We provide a primer for outcome assessment after amputation and assess the need for the additional development of novel instruments.

Mycotic Aneurysm Due to Lecleria Adecarboxylata: A Complication of Vertebral Osteomyelitis

Lecleria adecarboxylata is a Gram negative bacterium and member of Enterobacteriacae family. It has most frequently been isolated from lower wounds as a part of mixed flora. Mycotic aneurysms were first described by Osler and were mostly associated with infective endocarditis. We present the case of a male who presented with limb vein thrombosis, fatigue and a history of back pain. The computed tomography scan revealed a saccular aneurysm and combined vertebral osteomyelitis of the 3(rd) lumbar vertebra. An endovascular aneurysm repair with a tubed endograft was performed. Two years after the initial therapeutic management he developed buttock claudication due to ostial stenosis of both common iliac arteries and a “kissing stent” technique was performed. This is the first case described regarding Lecleria adecarboxylata as an etiologic factor of infected aneurysm development, its management and the short-term outcome.

The use of statin and antiplatelet medications has been advocated in patients with cerebrovascular disease as primary medical therapy and as an adjunct to carotid endarterectomy (CEA). Our goal was to assess the prevalence of preoperative statin and antiplatelet use and its effect on perioperative outcomes after CEA.
