Concept: Laser surgery


Small incision lenticule extraction or SMILE is a novel form of ‘flapless’ corneal refractive surgery that was adapted from refractive lenticule extraction (ReLEx). SMILE uses only one femtosecond laser to complete the refractive surgery, potentially reducing surgical time, side effects, and cost. If successful, SMILE could potentially replace the current, widely practiced laser in-situ keratomileusis or LASIK. The aim of this study is to evaluate whether SMILE is non-inferior to LASIK in terms of refractive outcomes at 3 months post-operatively.

ObjectiveTo analyze the complications and preliminary oncologic results of microscopic diode laser surgery.Study DesignProspective research.SettingAnkara Oncology Education and Research Hospital.Subjects and MethodsThis prospective study was conducted on a series of 64 patients with glottic carcinoma (Tis, T1, T2) treated with microscopic endolaryngeal diode laser surgery.ResultsFour patients had local recurrence (6.2%). Local control and larynx preservation rates were 93.8% and 100%, respectively, for all groups of patients. Two-year disease-free survival after primary surgery was 100% for the Tis group, 96.4% for the T1 group, and 89.7% for the T2 group. When considering anterior commissure involvement, 2-year disease-free survival after primary surgery was 100% for the AC0 group, 85.7% for the AC1 group, and 85.7% for the AC2 group.ConclusionOur study of microscopic diode laser resection of Tis, T1, and T2 glottic tumors showed similar oncologic results to previous reports about CO2 laser surgery. In anterior commissure tumors, both techniques had high complication and recurrence rates.

BACKGROUND AND OBJECTIVE: The thermal damage of the surrounding tissue can be an unwanted result of continuous-wave laser irradiations. In order to propose an effective alternative to conventional surgical techniques, photothermal damage must be taken under control by a detailed dose study. Real-time temperature monitoring can be also an effective way to get rid of these negative effects. The aim of the present study is to investigate the potential of a new laser-thermoprobe, which consists of a continuous-wave 1,940-nm Tm:fiber laser and a thermocouple measurement system for brain surgery in an ex vivo study. STUDY DESIGN/MATERIALS AND METHODS: A laser-thermoprobe was designed for using the near-by tissue temperature as a real-time reference for the applicator. Fresh lamb brain tissues were used for experiments. 320 laser shots were performed on both cortical and subcortical tissue. RESULTS: The relationship between laser parameters, temperature changes, and ablation (removal of tissue) efficiency was determined. The correlation between rate of temperature change and ablation efficiency was calculated. CONCLUSION: Laser-thermoprobe leads us to understand the basic laser-tissue interaction mechanism in a very cheap and easy way, without making a change in the experimental design. It was also shown that the ablation and coagulation (thermally irreversible damage) diameters could be predicted, and carbonization can be avoided by temperature monitoring. Lasers Surg. Endovenous laser ablation is an established alternative to surgery with stripping for the treatment of varicose veins. Ecchymoses and pain are frequently reported side effects of endovenous laser ablation. Device-related complications are rare but serious. We describe here an exceptional complication, necessitating an additional surgical procedure to remove a segment of laser fiber that had migrated into the pelvic cavity. Fortunately, severe damage had not occurred. This case highlights the importance of checking the completeness of the guidewire, catheter, and laser fiber after endovenous laser ablation.

Currently, the isotretinoin (13-cis-retinoic acid) package insert contains language advising the discontinuation of isotretinoin for 6 months before performing cosmetic procedures, including waxing, dermabrasion, chemical peels, laser procedures, or incisional and excisional cold-steel surgery. The use of operating laser poses a potential risk both to the patient and to theatre staff and so it is crucial that safety protocols and procedures are followed meticulously in every case - particularly in the setting of laser surgery in the airway. All existing guidelines and standards on laser safety highlight the importance of pre-use checks relating to aspects of the theatre environment and aspects relating directly to the patient. Good communication pathways have been highlighted as crucial for fire safety within theatre. The laser safety checklist allows the laser operator to make crucial checks on both environmental and patient factors prior to laser switch on and facilities the communication pathway for this in a clear and structured way.
